Phomecare Provide Community Day Program helps keep individuals active in the community, enhancing the quality of life for participants, and offering a respite for the care provider. Our disability day programs are hubs where programs are designed to support people with disability with a wide range of activities, engage in social connections.
The Community Day Program provides services to individuals with a wide range of challenges, including physical disabilities, diagnoses.
Transportation can be provided within Phomecare, and a meal is served at noon (food for special diets can be provided), though participants in the Modified Work Program must bring a bagged lunch.
We also provide a monthly caretaker Support Group, offering support and Care Givers of participants in the Community Day Program.
Hours of Operation:
9:00 AM to 4 : 00 PM, : Monday to Saturday
Access to the Community Day Program is through the Phome Care Health Region Patient Access Service
