Guidance on Defining Your NDIS Goals

Setting goals is a crucial aspect of the NDIS planning process, allowing individuals to tailor their plans to achieve the life they aspire to lead. These goals are unique and may encompass various categories, such as daily living, relationships, community participation, and employment.

When contemplating and establishing goals, there are no wrong answers; the key is to align them with personal aspirations and maintain motivation. Here are some tips to guide you:

  1. Engage with Your Family:

Communicate with your loved ones to refine and articulate your goals.

Your family can offer valuable insights, support your journey, and understand your preferences intimately.

  1. Reflect on Your Priorities:

Your NDIS plan is about your life, so ensure your goals reflect what holds significance to you.

Consider aspirations for increased independence, greater community participation, or pursuing employment.

  1. Consider the Future:

Contemplate your long-term aspirations, such as employment, independent living, or travel.

Recognize that goal achievement is an ongoing process; you can update your NDIS plan annually to adapt to evolving needs and ambitions.

  1. Identify NDIS Supports Needed:

Connect your goals with the necessary support. Assess if you require assistance with transportation, therapy sessions, or NDIS support coordination.

Be honest about the support you need to successfully pursue your goals.

  1. Consult Professionals:

Reach out to professionals for guidance if you feel overwhelmed or uncertain.

Your NDIS plan is designed to provide the necessary support for your well-being, and professionals can assist in maximizing its benefits.

Managing Disability Trusts – A Significant Responsibility

Imagine waking up each morning, relying on someone for assistance in getting out of bed, showering, and dressing. Depending on others to ensure your clothes are clean and presentable, and that your breakfast is not only nutritious but also enjoyable, with the possibility of needing help with feeding.

For individuals with disabilities, placing significant trust in others is a daily reality. P Home Care not only understands but deeply appreciates and acknowledges the immense trust placed in our hands.

We recognize that our staff play a pivotal role in determining the success of our clients’ days. Punctuality, attention to detail, and a consistent infusion of humor and excellent people skills are qualities our staff bring to their work every day.

At P Home Care, we prioritize delivering exceptional service and take pride in our ability to meet the diverse needs of our clients. If you seek a service provider genuinely invested in understanding and catering to your needs, ensuring your day starts on a positive note, don’t hesitate to reach out to us.

Supporting behaviour can significantly influence individuals with disabilities, as well as their friends and caregivers.

P Homecare creates tailored strategies that are responsive to the individual needs of the person receiving support. Strategies seek to lessen the necessity and application of restrictive practices as well as the incidence and effects of problematic behaviors.

Just as important to us is that while supporting behaviour change, we are also supporting the dignity and quality of life for the person requiring support.

We begin with a Behavioural Support Assessment carried out by our experienced Support Co-ordinators. The Assessment allows us to better understand the reason for the behaviour and what the person is trying to communicate through their behaviour. After obtaining information from individuals with disabilities, their families, and other pertinent sources, we will formulate customized strategies to enhance support for the client Strategies could include a Positive Behaviour Support Plan and/or a Skills Development Plan.

Plans will identify:

  • How to best support the person
  • How to lessen the need for the person to engage in behaviours of concern
  • Ways to respond safely if the person does use behaviours of concern

If there are any behaviours of concern that require replacement, our skilled Support Coordinators can facilitate access to the necessary skills for the individual to learn Occupational Therapy

  • Speech Pathology
  • Psychology
  • Counselling
